2023-24 School Year Physical and Paperwork Information (7th and 8th graders)

ALL Athletics paperwork needs to be submitted by August 31.  This includes the athletics physical, medical history and the ONLINE PARTICIPATION form. 

Coaches need a hard copy or printable copy of the Physical and Medical History forms.  The ONLINE PARTICIPAITON form is submitted online and none of the forms need to be printed.

If forms are not submitted by August 31, your schedule will be changed from Athletics to PE.
