Flexible Spending Accounts (FSA)

NEISD offers 2 types of flexible spending accounts: FSA Medical and FSA Dependent Care. Our FSA accounts are adminsitered by National Benefit Services (NBS).
FSA Medical:
A healthcare FSA, also know as FSA medical, is used to pay for eligible out-of-pocket expenses at the time of purchase or reimburse you at a later time. It can be used by you and your dependents.
These funds can be used for:
- Deductibles and Co-pays
- Medications
- Non-cosmetic dental work and orthodontia
- Glasses/Contacts
The IRS determines what expenses are eligible and ineligible and may change from time to time. Please visit NBSbenefits.com for more information and a full list of eligible expenses.

FSA Dependent Care:
A dependent care FSA is used to reimburse expenses related to the care of your eligible dependents while you work.
These funds can be used for:
- Day Care for children under 13 years old
- Before or after school care
Annual Contribution Limit:
Contributions into these accounts is limited by the IRS. Employees may contribute $300 to $3,200 to a FSA Medical account and $300 to $5,000 to a FSA Dependent Care account.
These are use it or lose it benefits, so plan carefully.

National Benefit Services, LLC (NBS) is our Cafeteria Plan Administrator and offer the following services:

- Auto substantiation on Debit Card Transactions
- Participant Web Access & Online Claim Submission
- Call center available to answer account questions M-F 6am-6pm
- 24 Hour Voice Response Unit to obtain basic account information
New NBS Debit Card:

Your NBS Debit Cards have been mailed! They will come in an unmarked envelope, please watch for them as they will arrive in the next 7-10 days. FSA accounts are effective January 1st through December 31st of the current year. Expenses for these accounts must be incurred by December 31, of the current year.

Claims

Claims can be processed using the debit card for FSA medical or by completing the FSA Claim Form above. FSA Dependent Care claims are processed via reimbursement using the FSA Claim Form above.
Complete the form, attach your receipts and submit to NBS via mail, email or fax.
