Supervision Waiver in English, in Spanish: parents of preK/Kinders list who student can be released to at bus stop
Bilingual Program - Bus Service Request in English, in Spanish: parents of students in bilingual progam request bus service
Pre K - Bus Service Request in English, in Spanish: parents of pre-K students request bus service
Bus Ride Permission Form: Submit request to your campus who will forward to transportation. Services may be approved, on a limited basis, to use alternate bus stop than what is assigned to home address.
Bus Ride Permission Form - Joint Custody: Submit request to your campus who will forward to transportation. Parents request approval to use alternate bus stop for reason of joint custody.
