  • Welcome to the NEISD All Things Aware Forms page. On this page, you will find information on how to attach an Accelerated Learning Plan form (meeting HB 4545 requirements) or an Accelerated Instruction Plan form through Eduphoria Aware and how to fill out the page with purposeful interventions to support students. This site is a collaboration between Curriculum & Instruction, Learner Support Services, Performance & Planning, and Student Leadership & Well-Being.

    General Forms Support

    Add a New Form in Aware (PDF) (Video)

    *Please note the Forms take about 10 seconds to process after clicking the save icon.  If using Chrome, you will see the message "Waiting for www.schoolobjects.com" in the bottom left corner.  The form will also refresh and return back to the top once it has saved.

     

    For questions about the content (how to fill it out, how to monitor progress, ideas for improvement, etc) of an ALP or AIP, please reach out to the content assistant directors.

     

    Monitor Updates on Forms

    *Please note, only users with the Campus Forms Administrator role will be able to do this. If you do not have thes capabaility (as shown in the video and help file), please contact PandA@neisd.net and request the Campus Forms Administrator role. 

    Who might need this role? LPAC Coordinator, 504 Coordinator, MTSS Coordinator, Principal and/or Assistant Principal, and anyone who monitors ALPs, AIPs, or Linguistic Accommodation forms in Aware.

ALP/AIP Samples

  • AIP (Accelerated Instruction Plan) Information

    Spring 2023 Update: All AIPs must be updated by the last day of school. Do NOT archive any form this year. The district will manage this process over the summer.

    Each year, you must create a new AIP Form in Aware for a student in need. Students who did not pass STAAR/EOC or did not take a STAR/EOC should NOT be assigned an AIP. Due to HB 4545, these students must have an Accelerated Learning Plan (ALP), which is a separate and brand NEW Form in Aware.

    Students who are struggling this year with academics, behavior, or in danger of not graduating (but were successful on STAAR last year or were in a non-tested STAAR grade level last year) should have an AIP created.
