Acanthosis Parent Notification Letter English, Huebner

Dear Parents/Guardians of Huebner Huskies,

Vision/hearing/acanthosis nigricans screenings will start to take place soon in the clinic. (PK, K, 1st, 3rd, and 5th, new students, and teacher requests). I will only notify parents/guardians with a telephone call for any concerns noted during the screenings and will send a referral letter home through the mail.

From the 23-24 Student Parent Handbook for Elementary Schools,

School Health Screening Programs:

Students are screened according to the Texas State Board of Education Rules and the school health programs mandated by the Texas Department of State Health Services. In each screening program, parents/guardians who receive a referral letter are asked to schedule an appointment with their physician/specialist. The referral letter should be completed and signed by the physician/specialist and returned to the school nurse.

Vision and Hearing Screening:

State law requires all PK, K, 1st, 3rd, 5th, all new students, and all students who are referred for special education evaluation be screened by the school nurse for vision and hearing abnormalities. Screenings are also done per the teacher’s request. The school nurse will contact and send a referral letter to the parents/guardians if any abnormality is noted.

Acanthosis Nigricans Screening:

All 1^st, 3^rd, & 5th grade students will be screened for a skin marker around the neck that potentially indicates high insulin levels in the body. High insulin levels create a potential risk for the future development of Type II Diabetes. Students who have the skin marker will also be weighed, measured, and have their blood pressure taken. The school nurse will contact and send a referral letter to the parent/guardian if medical evaluation is recommended.

If you do not want your child to participate in the Acanthosis Nigricans screening, please notify me.

It is possible to provide proof of recent screenings from a medical provider in the last 12 months to fulfill the requirement as well. (This also applies to 5^th grade girl’s spinal screenings. These are not scheduled on campus yet, but if they’re done at the doctor’s office, a parent may provide proof of that as well.)

Hi. My name is R. Michelle Titzman, BSN, RN. I am so happy to say I am Huebner‘s school nurse! I am from Corpus Christi, Texas. I received my BSN from UTHSCSA, now known as UT Health. I first worked for University Hospital in telemetry, MICU/CICU, and Endoscopy, and then for UTHSCSA as the Thoracic Nurse Coordinator. As my family grew, I was blessed to come work for NEISD. I have been a nurse for 29 years, and an NEISD school nurse for 21 of them. My varied experiences have guided me to my home at Huebner, and I am so happy here! (This is my 5th year here.)

I am married and have 2 children. My husband’s name is Allen and he is from San Antonio. My son, Devon, has graduated college and is working in accounting. My daughter, Lauren, is a senior in college and is studying finance. Our family pet is a long hair Chihuahua named Mojo. I enjoy spending time with family, being outdoors, taking walks, hiking, camping, water skiing, and crocheting.

Welcome Letter for Kindergarten Parents

Please read our letter to new parents regarding medications, immunizations and more.

Immunizations

State law requires students in Texas schools to be immunized against certain vaccine-preventable diseases. Getting your child vaccinated not only protects your child’s health, but also that of the community.

Medications on Campus

A reminder, per district policy, students are NOT ALLOWED to carry medications on campus (unless ordered by a physician and cleared with administration). This includes medicated lip balms, creams, ointments, cough drops, and throat lozenges. In addition, students are NOT ALLOWED to transport medication to the clinic. Parents need to bring in ALL medications to the clinic with physician orders.
