SB 571 Pre-Service Affidavit
Texas Senate Bill 571 established requirements for certain individuals who provide services to public schools. Under Texas Education Code §22A.055, individuals who will act as service providers for a school district may be required to complete a Pre-Employment/Pre-Service Affidavit before providing services.
Who Does This Apply To?
This process applies to individuals who meet the statutory definition of a service provider and have direct contact or interaction with students.

- Contractors
- Subcontractors
- Consultants
- Staffing providers
- Educational or instructional service providers
- Student support providers
- Other individuals providing services to or on behalf of the District
What Information Is Disclosed?
- Investigations, charges, adjudications, or convictions involving misconduct
- Licensing authority investigations or sanctions
- Placement on the Do Not Hire Registry
- Current or prior service to a public or private school
- Termination, discharge, or resignation in lieu of termination
Vendor Contract ≠ SB 571 Compliance
A purchase order or contract does not replace the SB 571 affidavit requirement.

An individual who fails to disclose information in the affidavit commits a Class B misdemeanor.
