Medication Information and Forms

Per NEISD Health Services, all medications for students must be kept in the clinic.

NO medications will be accepted unless they are:

1. In the original prescription container with the student’s name
2. Have a Medication Administration form signed by Dr.
3. The prescription label must match the order

Please note, this requirement is for ALL medications, including epi-pens and inhalers. Please help us in creating a safe school environment and have all documents and medications ready prior to the first day of school.
