What do I need to do to request a transcript?

The following information is required:

• Last name while enrolled in NEISD
• NEISD school last attended
• Last year of NEISD attendance
• A government or state-issued ID card, NO EXCEPTIONS (Currently enrolled students may present their student ID card for their proof of identity. Examples of acceptable identification are: driver’s license, state ID card, military ID, passport, or photo ID issued from a state or local government agency.)
• Signed authorization form for students under 18 years old
