Food Allergies

If your child has any foord allergies that you want the cafeteria to be aware of, you'll need to fill out a Meal Modification Request.  This needs to be completed by the parents and the physician.  You can find the forms on the District web page under Departments, School Nutrition, or ask me for a copy of the form.  Once it is completed, it comes back to the clinic, and then I send it to the School Nutrition Department.  If you have any questions, please feel free to reach out to me.
