Information for IKE Student Athletes:

• In order to be in Athletics, parent/guardian needs to complete all 3 of the following:
• Medical History and Physical PDF (to be scanned and emailed to girls or boys coach)
• Online Athletic Participation Form (to be completed and submitted online)
