General Clinic Information

Please help us keep all students safe with medication by:

• Not sending any medication to school with your child
• All medications must be dropped off in the clinic by a parent
• If your child will be self-carrying, we will need a parent at the initial drop-off, proper paperwork and training will be completed and then the student will be approved to self-carry on campus
• A new doctor's order is needed each year for medication
• Medications must be in original container with pharmacy label and not expired!

General

• Update phone numbers throughout the year on Parent Portal so we have up-to-date numbers in case of emergency
• Update the clinic with any changes in your child's medical status as appropriate
• Please return the Medical Information Sheet at the beginning of the year and contact us with any medical concerns
