    Information for UIL sport participation in the 2020-2021 school year:

    NEISD offers a voluntary insurance policy for all students. LEE Athletic Department highly recommends all of our athletes to purchase this! This policy will act as a secondary policy for those already covered by medical insurance, and as a primary policy for those who are not.     Due to COVID-19, the UIL has allowed Pre-Participation Physical Evaluations (PPE) done on or after May 1, 2019 to be valid until June 1, 2021. Student athletes that had their physical prior to May 1, 2019, or do not have a physical on file will be required to submit a new Pre-Participation Physical Evaluation.

    ALL students participating in UIL marching band and/or UIL athletic activities will be required to complete the online Athletic Participation Packet as well as the UIL Medical History Form. “Yes” answers to certain questions on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam, and receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches. Only those students with “yes” answers to those questions are required to receive written clearance through further medical evaluation and will be contacted provided correct contact information is on-file.

    Any participant in athletics or marching band, in any grade level, who has not previously completed a PPE, and been cleared for participation, will be required to complete the Medical History Form as noted above, as well as a PPE prior to participation in any UIL practices, games, performances, or matches.

    If the student athlete will be new to NEISD in the 2020-2021 school year, you will need to contact Josh Cross or Savannah Burks for further instructions on how to complete the forms.
