Registration

 

Items needed for registration are listed below.   

 

1.  Parent or legal guardian must register student** (If not natural parent, guardian must  clear through Pupil Personnel at Central Office)

2.  Official Birth Certificate - can not accept hospital certificate

3.  Proof of Residence (Entire CPS, Water Bill or rental or lease agreement - driver's license is not acceptable.)

4.  Immunization Record (Shot record must be current)

5.  Social Security Card 

6.  Last report card or withdrawal form from previous school (if one was attended)

**  Photo ID required of person registering student
