ITEMS NEEDED FOR REGISTRATION

The following items are needed for registration:

1. A parent or legal guardian must register the student.
( If not natural parent, guardian must clear through Pupil
Personnel at Central Office.)

2. Proof of Residency
(Current utility bill or current lease/contract on apartment
or house. Driver’s license is not acceptable.)

3. Legal Birth Certificate

4. Immunization Records
(Shot record must be current: i.e. booster in DPT and polio
after age 4)

5. Student’s Social Security Card
( Copy needed for permanent records)

6. Last report card or withdrawal form from previous school
(If appropriate)
