| New Student Registration |

| We will enroll new kindergarten students (MUST BE 5 YEARS OF AGE ON OR BEFORE 9/1/11) as well as students in 1st -5th grades that are new to NEISD. Please see the requirements below. We look forward to our new Stone Oak Bobcats!

- Only completed registration packets will be accepted
- Required Item for Registration
- ·Picture ID of the parent registering the student
- · Student's Birth Certificate
Certified, State issued (Copies or hospital records cannot be accepted.)
- · Student's original Social Security Card (Copies cannot be accepted.)
If one is not available, please see Regina Perez, the Data Processor, for further instruction.
- · Student's Current Immunization Records
Students who are not fully immunized will not be registered.

- · Proof of Residency - any one
of the following will qualify:
- Lease Agreement - must be signed &
the student's name must be on an apartment lease agreement.
- Current CPS bill - call 353-2222 for
immediate billing if necessary
- Current Bexar Met bill - call 354-6400 for
immediate billing if necessary
- Advanced Enrollment Form - Persons not
currently residing within our attendance (ie. house is under construction)
are required to have an advanced enrollment form. Please call the district
office at 804-7070 for additional information & to schedule an
appointment with the Pupil Personnel Department to obtain the proper form.
- · Previous school address & withdrawal form or last
report card (for first grade and up)
