Registration Information
The following items are required in order to register your new student at Roan
Forest
**Parent or legal guardian must register student
**Official Birth Certificate
**Proof of Residence (CPS, Water Bill or current
lease/contract on apartment or house. Driver's license
is not acceptable.)
**Immunization Record (Shot record must be current)
**Social Security Card
**Last report card or withdrawal form from previous
school (if one was attended)
** Photo ID required of person registering student
**Social Security Card (If one is not available or parent does not want this number used, a state ID will be assigned.)
