IMMUNIZATIONS |
All students entering school for the first time are required to submit evidence of the required immunizations as specified by the Texas Department of Health and district policy at the time of enrollment with the birth certificate and proof of residency and other require-ments. Any student not presenting such documents shall not be enrolled or attend classes until such documented proof of complete immunization is submitted. Students will not be allowed to start school without the completed immunizations, or having begun the required immunizations, continue to receive the immunizations as rapidly as is medically feasible. |

EXCLUSION FOR NONCOMPLIANCE |
If student immunization records are not in state compliance, policy necessitates exclusion from the school campus of those students not in compliance until such time as their records are current. The following procedures will be used: 1. A letter of notification is to be sent to the parent/guardian. 2. Their absences will be excused for three days only. 3. If a student has not cleared the immunization record within three days after exclusion, notify the department of pupil personnel services. |

MEDICAL AND RELIGIOUS EXEMPTIONS |
Students shall submit evidence as specified in the law. Exclusions for medical and religious reasons may be granted as specified by the Texas Department of Health. Exclusions for religious reasons may be granted if the immunizations conflict with the tenets of an organized religion as specified in the law. Contact Health Services for information and to request forms for these exemptions |

EMERGENCY CARD |
Administrative guidelines for emergency card and illness care: 1. Each student is to have a current emergency card on file. 2. Cards are to be completed by the parent(s) or guardian at the beginning of each school year and kept in the school clinic. 3. A system should be in place at each school for obtaining an emergency card for each student. 4. Parents are to be contacted by the school nurse for those students identified with a health problem and procedures for care for each student. |
