NEISD Immunization Requirements

• Proof of residency
  Copy of entire current utility bill showing address, month of bill and bill amount, or lease agreement

  
  

• Current immunization record
  See immunization requirements web page by NEISD Health Services

  
  

• Original birth certificate

  
  

• Social Security Card
  If one is not available or parent does not want this number used, a state ID will be assigned

  
  

• Report card from previous school (if applicable)
