School Health Care for Students

Medication Administration Request
If a student has a health concern and requires medication during the school day, the prescribing physician must complete a Medication Administration Request form. Students requiring procedures/treatments during the school day will be required to submit the Physician/Parent Request for Administration of Procedures/Treatment form. These forms must be completed annually and whenever there is a change of medication, dose, procedure, and/or treatment. The school nurse will contact the physician as necessary for information regarding student health concerns, medication, and procedures/treatments. Health concerns and related information will then be shared on an as needed basis with appropriate school staff to provide appropriate care for your student.

Screenings

Screening programs are conducted in accordance with State Law and the Texas Department of Health. Parents are contacted and a referral is sent by the school nurse, should a problem be noted. The child’s physician/dentist should return a report to the school nurse within 30 days.

School Screening Programs Conducted in NEISD

Vision and Hearing Screening: Students in Pre-K, K, 1st, 3rd, 5th and 7th graders are screened annually, as well as all new students to Texas and referrals for Special Education.

Spinal Screening: Students are screened for scoliosis in the 8th grade.

Acanthosis Nigricans Screening: Each student in the 3rd, 5th and 7th grades is screened for a “skin marker” around the neck resulting from a higher concentration of insulin in the bloodstream, which creates a potential risk for the development of Type 2 Diabetes. The weight and blood pressure will be taken on those students identified in the program.

Exemptions to Screenings are allowed as specified in State Regulations, please contact your School Nurse for information.

Should you need any information or assistance concerning your child’s health, development and/or screening, please call the school nurse at your child’s school.
