apartment lease
(with the student's name on the lease)
If living with another family, owner or lessee must also
be present to fill out proof of residence
form that must be notarized with a valid picture ID.
3. Immunization Record (shots MUST BE current )
4. Social Security Card for student
5. Withdrawal form and/or last report card from the last school attended.
If a junior or senior, an academic transcript is also needed. The
parent is responsible for obtaining this information.
6. Current address of the last school attended
7. Birth Certificate
8. If you did not finish school last year, you MUST see
an administrator
9. As of September 1, 2001, persons registering students need to show
picture proof of
identification and their birth date
