VERIFICATION OF ENROLLMENT (T.E.A. FORM) FOR DRIVER’S PERMIT OR LICENSE

All students under the age of 18 years must obtain a Verification of Enrollment form
each and every time they apply for or renew a driver’s license or permit.
 (The verification of enrollment is called a T.E.A. form simply because it is required by Texas Education Agency)

IF A STUDENT RECEIVED AN N/C (NO CREDIT) FOR ATTENDANCE DURING THE SEMESTER PRIOR TO THE CURRENT SEMESTER THEY ARE NOT ELIGIBLE FOR A T.E.A. FORM AND CANNOT OBTAIN EITHER THE DRIVER’S LICENSE OR THE DRIVER’S PERMIT. 

Summer school attendance is not counted for a T.E.A. form.

At Madison H.S. the bookkeeper issues the T.E.A. forms and students must request the form by filling out a simple request form in the bookkeeper’s office. They MUST have their student ID number and their Social Security number. 
The form will not be processed without these numbers.

The student will request the form in the bookkeeper’s office and if eligible will receive it at the end of the NEXT school day. 
School policy is that NO exceptions will be made to this schedule.

Complete this form and email as an attachment to the bookkeeper, Ann Weddle, at aweddl@neisd.net The T.E.A./verification of enrollment form will be ready in the bookkeeper’s office the NEXT day after school. NO EXCEPTIONS ARE MADE TO THIS TIMELINE.

The The T.E.A./verification of enrollment form must be an original and cannot be FAXed or emailed; it must be picked up at school in the bookkeeper’s office.
