As the school nurses, the health and well being of our students is our highest priority. Your assistance with the procedures listed below is very important in helping us keep your student and our campus healthy and safe. Please read the following and feel free to call us or stop by the clinic at any time if you have questions.

All students are required to have a current emergency card on file in the clinic. This information is a vital part of your student's records. If an injury occurs at school, first aid will be given. If additional medical care or evaluation is necessary, we will attempt to contact a parent. If we are unable to reach a parent, we will contact one of the alternate adults listed. It is very important to fill this card out completely. Please notify the clinic of any changes made throughout the year. Students will only be released to people listed on the emergency card. So please list all adults who might be available during the year. Please return these as soon as possible!

If at any time during the school year your student is on crutches or in a wheelchair, please provide a doctor's note and an elevator key will be issued. A $10 deposit is required and will be returned upon receipt of the key.

All medications must be kept in the clinic. All medications must be brought to school & taken home by a parent! Prescription medication remaining in the clinic for over 10 days must be accompanied by a "Medication Administration Request" form signed by both the parent and the physician. While waiting for the physician's signature, medication will be administered for 10 days with a parent signature. All medications must be in the original container with correct dosage information on the label. Over-the-counter medications may be left in the clinic all year with the "Medication Administration Request" form signed by the parent and the physician. Short-term medications may be left in the clinic for 10 days with a parent note. If your child plans to carry an inhaler, an epi-pen or diabetic equipment, all required paperwork must be filed in the clinic and a label will be applied to the medication to show proof of permission. If you or your student would like to speak to us concerning a health problem or concern, please contact us, as we enjoy working closely with parents and students. If we have a concern regarding a student's health, we will contact a parent or guardian. We look forward to working with each of you this year.
