ITEMS NEEDED FOR
REGISTRATION
1. A parent or legal guardian
must register the student.
**If
not natural parent, guardian must clear through Pupil
Personnel at Central Office.
2. Proof of Residency **Current utility bill or current lease/contract on apartment or house. Driver’s license is not acceptable.
3. Legal Birth Certificate
4. Immunization Records www.neisd.net/health/immunization.html
**Shot
record must be current: i.e. booster in DPT and polio after
age 4
5. Student’s Social Security
Card
**Copy needed for permanent records
6. Last report card or withdrawal form from previous school (If appropriate)
