Nurse's Welcome Letter

August 14, 2006

Dear Parents/Guardians and Students:

Middle school is a time of many changes. Our goal at Garner is to help our students make healthy lifestyle choices including proper nutrition, adequate rest, and regular medical/dental care. In order to foster a sense of responsibility in our students and to ensure a safe and orderly Clinic environment, the following North East Independent School District policies are followed:

1. Students must have a pass from their teacher to come to the Clinic.

2. Students are asked to care for injuries that occur at home before coming to school. Injuries that occur at school will be assessed and treated with basic first aid. You will be contacted if immediate care or follow-up is required.

3. Students with fever must stay home. Students should remain home for 24 hours after the fever has broken.

4. Students who wish to contact parents because of illness should come to the Clinic to call rather than use the pay phone.

5. If your child has a special health concern such as asthma, allergies, diabetes, etc. AND/OR if daily medication is needed throughout the school year, your child’s physician must complete a Medication Administration Request each school year. The form is available in the Clinic.

6. Medications that are to be given daily during the school year will NOT be accepted in the Clinic without the Special Health Problem form signed by the physician AND a parent/guardian.

7. NEISD policy prohibits students from carrying medications on the bus, in their lunch boxes, and on their person. All medications must be brought to the Clinic by the parent/guardian. PLEASE NOTE THIS POLICY.

8. Medication MUST be in the original prescription bottle/container. Ask your pharmacist for two labeled bottles – one for home and one for school. There is no extra charge for this service. Medication in foil, baggies, envelopes, etc. will not be given.

9. Parents of students on daily medications are asked to monitor the amount of medication that is brought to school and make a note on your calendar when a refill is needed. This ensures the continued administration of daily medications. Due to the volume of medications, the Clinic staff is not always able to make reminder calls.

10. Homeopathic medications and medications purchased outside the United States will not be administered.

11. Parents are encouraged to arrange a conference with the school nurse if their child has a special health need so that a school health plan can be initiated. Good communication between home and school makes your child’s transition much easier.

12. NO Shots, NO School, NO Kidding! Computer generated immunization notices are mailed to parents several times each year. If you receive a notice, please note the compliance date on the letter and have you child immunized prior to that date. Written proof of current immunizations must be on record in the Clinic before 8:15 AM on the compliance date noted in the letter. Students who are not current on their immunizations will not be allowed to attend classes until the required immunization has been received AND proof is on file in the Clinic.

Please discuss this information with you student and keep it as a reference. If I can be of assistance to you or your child, please feel free to contact me. Best wishes for a healthy and successful school year.

Sincerely,

Jeanine Crumrine, R.N.,  B.S.N.
