When registering your child, the following documents will be needed:

1.  Original Birth Certificate issued by the STATE

2.  Original Social Security Card

3.  Proof of Residence
     (Copy of lease with child(s) name or current City Public Service)

4.  Current Immunization record

5.  Photo ID of person registering the child
