| All medication intended for students shall be kept in the school’s clinic. Short-term prescription medication will be administered according to the instructions on the prescription label, and with written authorization from the parent. Long-term medications to be given all year or for emergency situations require a Medication Administration Request form signed by the doctor and written authorization from the parent. |
Children are not to transport medication to or from school.

| Medication must be brought to the clinic by the parent or another adult. Non-prescription medication is permitted on a limited, temporary basis requiring written parent authorization. Medication will only be accepted in its original container. Prescription medication will only be accepted in a properly labeled pharmacy bottle. Clinic personnel will administer all medication. |
