State Law and NEISD policy require medically validated up-to-date records of immunizations to be on file for all students.  Immunization records must be as complete as medically feasible.  To claim exclusions for medical reasons or reasons of conscience, including a religious belief, please contact the school nurse or NEISD Health Services at 804-7147.

Immunization Requirements

3 Years of Age

4 Doses    DTP, DTaP (Diphtheria, Tetanus, Pertussis)

3 Doses    IPV (Polio)

1 Dose      MMR (Measles, Mumps, Rubella) on or after 1st birthday

1 Dose      Hib (Haemophilus influenzae type b) on or after 15 months OR primary series with booster on or after 1st birthday

3 Doses    Hepatitis B

1 Dose      Hepatitis A (1st Dose on or after 2nd birthday)

1 Dose      Varicella (chickenpox) on or after 1st birthday OR a statement of date of chickenpox illness  

(See Chart)      PCV7 ( Pneumococcal Conjugate Vaccine)

4 Years of Age

5 Doses    DTP, DTaP (Diphtheria, Tetanus, Pertussis) 5th Dose on or

                after 4th birthday OR 4 Doses with 4th Dose on or after 4th

                birthday

4 Doses    IPV (Polio) 4th Dose on or after 4th birthday OR

                 3 Doses with 3rd Dose on or after 4th birthday

Kindergarten and First Grade

                 after 4th birthday OR 4 Doses with 4th Dose on or after 4th

                 birthday

Students who are 7 years of age or older:

          3 Doses Tetanus-Diphtheria containing vaccine, one of which

      must have been received on or after the 4th birthday.    

     Pertussis vaccine is not required.

4 Doses   IPV (Polio) 4th Dose on or after 4th birthday OR

               3 Doses with 3rd Dose on or after 4th birthday

2 Doses   Measles (1st Dose on or after 1st birthday)

      last dose of DPT/DTaP/DT/Td

                   Not required for students 18 years and older

2 Doses     Measles (1st Dose on or after 1st birthday)

ADDITIONAL INFORMATION

bullet Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirements for ALL vaccines.
bullet Grades 1 - 12 for DTP, DTaP, DT and Polio:   The calendar month prior to or of the 4th birthday is valid.
bulletPolio:   No dose is required on or after age 4 if any combination of 4 doses of both OPV and IPV were used.  However, if the series is comprised of all IPV or all OPV, then a dose is required on or after age 4.
bulletSerologic confirmations of immunity to measles, rubella, mumps, hepatitis A, hepatitis B or varicella are acceptable.  Evidence of measles, rubella, mumps, hepatitis A, hepatitis B or varicella illnesses must consist of a laboratory report that indicates either confirmation of immunity or infection.

  Exemptions

bulletMedical reasons - Exclusion from compliance includes medical reasons (a statement from an M.D. or D.O., duly registered and licensed to practice medicine in the United States, is required), stating that in the physician's opinion that the vaccine required is medically contraindicated or poses a significant risk to the health and well-being of the child or to any member of the child's household.  Unless it is written in the statement that a life-long condition exists, the exemption statement is valid for only one year from the date signed by the physician.
bulletReasons of Conscience - Conscientious Exemptions, including religious beliefs, must be obtained from the Department of State Health Services and submitted to the school nurse for immunization exemption upon enrollment or at the time exemption is requested.

Provisional Enrollment

bulletProvisional Enrollment:  A student may be provisionally enrolled if he/she has begun the required vaccine series and is receiving subsequent immunizations as rapidly as medically feasible. 
bulletIf a student is transferring from another Texas school, he/she may be enrolled for 30 days while awaiting the transfer of the immunization record
bulletChildren of active duty military families who are moving to Texas from out-of-state and are awaiting the transfer of immunization records may be provisionally enrolled for 30 days. 
bullet A student who is homeless, as defined by the McKinney Act, shall be admitted temporarily for 30 days if acceptable evidence of vaccination is not available.
